I was able to purchase a replacement electrolytic power supply capacitor at Radio Shack and got it working again. It's been fine since then.

Super simple setup
This is a sweet switch and it was really easy to install and use.
I have about 10 of these in service at different small business client sites, been using for about two years without any problems. Some have replaced several failed devices from other manufacturers.
Works reliably, can be rack-mounted (adapters included) or surface-mounted (non-slip pads included); Not a full rack-width wide device, so it's compact when used on a shelf or tabletop
All lights are green; have to study them for a quick moment to figure out what they're saying
